Hospice nurses are specialized professionals dedicated to end-of-life care, offering continuous, hands-on nursing support while ensuring a dignified passing. Each hospice patient is assigned a case manager nurse, who works closely with the entire care team to develop a personalized care plan and oversee the patient’s overall well-being.

Case managers are available around the clock, providing support to both patients and their families at any time of day or night. A Field RN or LVN is also assigned to visit the patient one to three times a week. They are responsible for performing assessments, taking vital signs, managing pain and symptoms, dressing pressure ulcers, and ensuring the patient’s comfort, focusing on comfort care rather than curative treatments.

Hospice aide services provide personal care and support to patients, assisting with activities of daily living such as bathing, dressing, grooming, and toileting. They also help with light housekeeping, meal preparation, and ensuring the patient’s comfort and safety. Hospice aides work under the supervision of a registered nurse and are dedicated to maintaining the dignity and well-being of the patient while providing compassionate, hands-on care.

The hospice doctor is available at all times to provide medical care and support to patients who are terminally ill and in the final stages of their illness. They are responsible for prescribing medications to manage symptoms and enhance the patient’s quality of life, as well as certifying and recertifying patients through face-to-face visits in their homes. The hospice doctor assigns a terminal diagnosis and ensures accurate, comprehensive information is provided regarding the patient’s condition, prognosis, and treatment options. They work closely with the case managers to coordinate and oversee the patient’s care plan.

Our medical social workers assist patients and their families in navigating the social and emotional challenges that often accompany the end-of-life journey. They are trained to offer compassionate, non-judgmental emotional support, while also connecting families to community resources to ensure patients receive the necessary medical care and additional support.

Hospice chaplains are trained to offer spiritual and emotional support to patients and their families, providing care tailored to their individual religious beliefs or spiritual practices. Regardless of faith, they offer comfort and guidance during this challenging time. Additionally, hospice bereavement counselors assist families in coping with the grief and loss that comes with the death of a loved one, helping them navigate the emotional complexities of their mourning process.

Volunteers in hospice care are specially trained to offer companionship, transportation, entertainment, and other types of assistance to patients. Under the guidance of a Volunteer Coordinator, they receive thorough training to ensure they can provide compassionate, high-quality care. Their support helps enhance the patient’s quality of life, offering both practical help and emotional comfort during this time.

ProAngels Inc Hospice provides comprehensive coverage for all medications related to the hospice diagnosis, as well as medications specifically designed to manage pain and alleviate symptoms.
